Student Travel and Health Declaration Form
The information filled in the declaration form must be correct before entering the scope of the hospital.
In order to better understand the health status of each student, and effectively prevent " To protect the health of each student due to the spread of the novel coronavirus, our school plans to collect information on the health status of each student.
Parents are requested to fill in the following "Health and Travel Declaration Form" for their students so that the school can make appropriate arrangements, thank you.
Under the following circumstances, the school reserves the right to deny the student entry to the campus and ask the student to stay at home for rest.
1) If the student has fever or cough symptoms
2) If the student's address has a confirmed case of novel coronavirus/close contact of the student or the student himself is under home quarantine
3) Compulsory testing issued by the government has not been completed
Students in the following circumstances will be suspended from returning to school (the branch will follow up individually):*
1) There is a confirmed case in the building/estate where the student lives
2) There is a confirmed case in the kindergarten/primary school the student is attending
3) If the student has a confirmed case in the company/school of a relative
4) Subject to compulsory testing issued by the government and has completed and obtained a negative result certificate
Personal data will be used, processed and stored by the hospital in accordance with the Privacy Policy and the requirements of the Health Center.
