學生接送登記表

所填寫的資料必須正確

1. 如家長／監護人忘記帶接送證，需填寫接送登記表方可與您的兒童離開學院。

    If parents / guardians forget to bring the Pick Up Card, it is need to fill the ID registration    for picking up the student(s).

2. 如接送證遺失、毀損/額外做證，請向校務處以$50/@申請補發，以確保學生安全。

    If the Pick Up Card is lost or ruined or need extra ones, please contact KINGS School Office for an extra one with $50 for the safety of the student(s)
